HU-CCR Cancer Track: PI Interest Form

To indicate your interest in hosting a HU-CCR Cancer Track student for a rotation, please complete the below form in its entirety.

Cynthia Gordon, Program Administrator, will follow up with you regarding next steps.

Please provide any dates between September 2024 and September 2025 when you will NOT be able to host a HU-CCR Cancer Track rotation student. (Rotations are typically 6-10 weeks.)
Should you and the HU-CCR Cancer Track student decide to move forward with dissertation research in your lab, please provide the earliest date you would have a CRTA4 slot (or higher) available.
Have you completed the Annual Implicit Bias Training (NCI OWPD)?
If no, please register for the next available training. The Annual Implicit Bias Training can be found on the learning portal.
Have you completed the Difficult Conversations, Conflict, and Feedback Training (NIH OITE) or Crucial Conversations Workshop (NCI OWPD)?

If no, please register for the next available training. Please contact Lori Conlan for the next available training date. The Crucial Conversations Workshop can be found in the OWPD catalog.
Have you completed the Training/Mentoring Grad Students and Postdocs (NIH OITE)?
If no, please register for the next available training. Please contact Lori Conlan for the next available training date.
